Carewatch meets Government drive for outcome-based care with new monitoring tool

The UK’s national provider of local home-based care and support, Carewatch Care Services Ltd, has launched a new outcome-based monitoring system to meet the Government’s push for a person-centred approach to care. The system has been designed to put the individual receiving care and support at the centre of the service, giving them choice and control which mirrors the objectives set out in the white paper, Our Health, Our Care, Our Say.

The bespoke Outcome Monitoring System has been developed by Carewatch, to measure and report on individual needs, their journey to independent living and the part Carewatch plays in ensuring their goals are met. The system signals a move away from traditional “task” based, more rigid service provision to a more individual approach which allows Carewatch to deliver what the service user wants, as well as being able to monitor and review progress and make the most effective use of the service Carewatch provides.

By designing an individual care plan based around the needs and goals of the person, and using the system to monitor and review progress, it is possible to identify the most effective ways in which the service user can become more independent within their home. In addition, the Outcome Monitoring System will help Carewatch provide precise information to the Care Quality Commission (CQC) for ongoing audits.
